Blueprint Quiz/survey — Protocol quiz (11 screens, Okendo)

Why it works

Eleven screens that auto-advance on tap, opening with the least sensitive question available and never asking for anything identifying until the very end. Two things make it worth studying. First, the personalization is real: walking two disjoint personas returned overlapping-but-different baskets, and re-walking one from a clean browser context returned an identical result, so the engine is rule-based rather than decorative. Second, sex at birth is a router — answering Female inserts two screens (pregnancy, menopause) that the male path never sees, and answering yes to pregnancy produces a liability disclaimer rather than a disqualification. The funnel never turns anyone away.

    "How old are you?" — six bands from 18-24 to 65+. The cheapest possible first ask: no typing, no identity, one tap, and the progress bar immediately moves.

    "What was your sex at birth?" — Male / Female / Prefer not to say, each a photo tile. This is the router. Female opens two additional screens; the other two answers skip straight past them.

    "Are you pregnant?" — female branch only. A supplement brand asking the safety question it has to ask, placed early rather than buried in a footnote.

    Answering "Yes" produces this full-screen disclaimer — "you acknowledge and agree that any product recommendations are not specifically intended for use during pregnancy, and you assume full responsibility" — with a Next button. It is a liability transfer, not a gate: the quiz continues and still sells.

    "Are you perimenopausal or postmenopausal currently?" — Yes / No / I'm not sure. The second female-only screen, and the only question in the quiz offering an explicit "not sure" escape.

    Country — 25 options, the only dropdown in the quiz and the only screen requiring an explicit Next. It also ships a typo: "Czeck Republic".

    Dietary preferences — Plant-based / Keto / Paleo / None, multi-select. Worth noting against the output: the plant-based persona was still recommended Omega-3.

    "How confident are you in your understanding of wellness protocols?" — a sophistication read (Extremely / Somewhat / Not confident yet) that has no visible effect on the recommendation. It profiles the buyer for segmentation rather than for the basket.

    Format preference — powder, capsules, bars, no preference. This one is visibly honored: the capsule persona got capsules, the powder persona got a $79.80 protein powder, the single most expensive item recommended to either. A preference question that also raises order value.

    Lifestyle — ACTIVE / BUSY / BALANCED. Note that every option is flattering; "BUSY: low movement, high mental stress" is the closest the quiz comes to letting anyone admit a problem.

    The screen that does the work: eleven icon tiles, choose up to five. This answer selects three of the five recommended products — the only place in the quiz where a choice visibly changes what the visitor is shown.
