Salesforce Opt-in/lead form — Eight fields, phone required

Why it works

First name, last name, job title, company, company size, country, email, phone — all required. It is the same form, built from the same component, on every Salesforce gated offer: a report, a calculator or a three-minute demo video all cost the same eight answers. The country is filled in from the visitor’s location, and changing it changes the consent.

    As a visitor from Serbia sees it: country preset, phone code +381, and an unticked box, “Yes, I would like to receive marketing communications regarding Salesforce products, services, and events.”
  2. Salesforce — Eight fields, phone required (screen 2)
    The same form with the country set to United States. A required State/province field appears, and the marketing box is gone: only “We value your privacy”. A US lead is opted in by filling the form.
