Checkout/order form
Prose Checkout/order form — Checkout, account details
Why it works
Prose's checkout is headed "Love your first order (or it's on us)". The summary spells out what the cart only implied: "Ships every 4 weeks", and the 60% is "off first subscription order", so the discount applies to the first box only. Phone number is required here, with two SMS consent boxes beneath it. Shipping and payment follow on later screens.
$156.00 − $93.60 "60% off first subscription order" = $62.40 · ships every 4 weeks
